First-Time Botox: What to Know Before Your First Appointment

How Botox works and what it treats

Botox is a purified form of botulinum toxin type A that blocks nerve signals to targeted muscles, reducing the repeated muscle contractions that cause dynamic wrinkles — most commonly forehead lines, frown lines between the brows (glabellar lines), and crow's feet around the eyes. It does not add volume; for hollowed or sunken areas, a dermal filler is used instead.

The FDA first approved Botox Cosmetic for glabellar lines in April 2002, and later expanded approved cosmetic indications to include crow's feet (2013) and forehead lines (2017).

What your first appointment looks like

Expect a short consultation where the provider assesses your facial muscle movement at rest and while animated (frowning, raising eyebrows), discusses your goals, and marks injection sites. The injections themselves take only a few minutes using a very fine needle; most patients describe it as a quick pinch rather than significant pain, and topical numbing cream is available at most providers if you're needle-averse.

You can resume normal activity immediately. Providers typically recommend staying upright and avoiding vigorous exercise or lying face-down for about 4 hours, to reduce the small risk of the product migrating from the injection site.

Timeline and what to expect from results

Onset is gradual — most people notice initial softening within 3–7 days, with full effect visible around day 14. Results typically last 3–4 months as the muscle activity gradually returns; first-time patients sometimes see slightly shorter duration than repeat patients, since muscle memory and consistent treatment can extend results over time.

Mild bruising, redness at the injection site, or a temporary headache in the first 24 hours are the most commonly reported side effects. A rare but recognized risk is temporary eyelid or brow drooping (ptosis) if the product spreads beyond the target muscle — this is uncommon with an experienced injector and resolves as the treatment wears off.

Questions to ask before you book

Ask who is actually performing the injection (physician, nurse practitioner, physician assistant, or registered nurse) and their specific injectable training. Ask whether the product is genuine Botox Cosmetic (as opposed to Dysport, Xeomin, or Jeuveau — all legitimate but distinct neuromodulators with different dosing) and how many units your treatment area typically requires, since pricing is usually per-unit.
