How Biostimulators Work

Biostimulators trigger a controlled inflammatory response in the dermis. The product particles are recognized as foreign by the immune system, which recruits fibroblasts to the area. Those fibroblasts produce new collagen — primarily type I collagen — around the product particles. As the product degrades over months, the newly formed collagen persists in its place.

This is fundamentally different from hyaluronic acid (HA) fillers, which work by mechanical volume — the gel physically occupies space under the skin and can be dissolved with hyaluronidase enzyme if needed. Biostimulator results appear gradually over 3–6 months as collagen builds; HA filler results are immediate.

How Long Does Sculptra Last?

Sculptra (poly-L-lactic acid) has pivotal trial data supporting duration of 2–3 years. In the landmark study by Vleggaar et al. (2011, Journal of the American Academy of Dermatology, n=278), patients demonstrated sustained facial volume correction through 25 months. The collagen produced is the patient's own type-I collagen — the product itself degrades completely in 9–12 months, and the collagen persists after that.

Most patients require 2–3 treatment sessions spaced 4–8 weeks apart. Results are not immediate — patients typically see gradual improvement beginning at 3 months and peaking at 6 months.

When to Choose Biostimulator vs Filler

Choose a biostimulator when treating large surface areas (temples, cheeks, jawline, body), when the patient is comfortable waiting 3–6 months for peak results, when the goal is natural gradual improvement, and when the patient wants to invest in long-term collagen instead of repeated maintenance sessions.

Choose an HA filler when precise immediate volume is needed (lip augmentation, acute facial asymmetry), when reversibility is important (first-time patient, high-risk anatomy), or when the treatment area is small and targeted.

Many experienced injectors combine both approaches — biostimulators for the broad foundation of volume and collagen, HA fillers for precise contouring in specific areas.

What to Know About Bellafill

Bellafill is unique among biostimulators because the PMMA (polymethylmethacrylate) microspheres do not degrade — making it effectively semi-permanent. It is the only FDA-approved filler with a 5-year clinical study, and it holds FDA approval for both nasolabial folds and acne scars. Because it cannot be reversed, it requires a skin test for bovine collagen (the carrier gel) prior to treatment, and patient selection should be conservative.

Frequently Asked Questions

What is the difference between biostimulators and dermal fillers?+

Biostimulators (Sculptra, Radiesse, Bellafill) work by triggering your body to produce new collagen. They do not add gel volume — results develop gradually over 3–6 months as collagen builds. Hyaluronic acid fillers (Juvederm, Restylane) work immediately by adding gel volume that can be dissolved with enzyme if needed.

How long does Sculptra last?+

Sculptra results typically last 2–3 years based on pivotal clinical trial data. The poly-L-lactic acid particles degrade in 9–12 months, but the collagen your body produces around them persists. Most patients require 2–3 sessions to achieve optimal results.

How long does Radiesse last?+

Radiesse (calcium hydroxylapatite) typically lasts 12–18 months. It provides immediate volume while simultaneously triggering collagen production. The CaHA particles gradually absorb into the body, leaving collagen in their place.

What is Bellafill and how long does it last?+

Bellafill is a semi-permanent filler containing PMMA (polymethylmethacrylate) microspheres in a bovine collagen carrier gel. It is the only FDA-approved filler with a 5-year clinical study. The PMMA microspheres do not dissolve, making Bellafill effectively permanent. It is approved for nasolabial folds and acne scars.

How does the cost of biostimulators compare to HA fillers?+

Biostimulators typically cost more per session — Sculptra $800–$1,200 per vial, Radiesse $600–$1,000 per syringe — but last significantly longer. When calculated on a cost-per-year-of-correction basis, biostimulators are often more economical than HA fillers for large-area treatments.

Can biostimulators be reversed?+

No. Unlike hyaluronic acid fillers which can be dissolved with hyaluronidase, biostimulators cannot be reversed. Sculptra, Radiesse, and Bellafill all require careful patient selection and conservative treatment because there is no antidote if the patient is unhappy with results.
