How Biostimulators Work

Biostimulators trigger a controlled inflammatory response in the dermis. The product particles are recognized as foreign by the immune system, which recruits fibroblasts to the area. Those fibroblasts produce new collagen — primarily type I collagen — around the product particles. As the product degrades over months, the newly formed collagen persists in its place.

This is fundamentally different from hyaluronic acid (HA) fillers, which work by mechanical volume — the gel physically occupies space under the skin and can be dissolved with hyaluronidase enzyme if needed. Biostimulator results appear gradually over 3–6 months as collagen builds; HA filler results are immediate.

How Long Does Sculptra Last?

Sculptra (poly-L-lactic acid) has pivotal trial data supporting duration of 2–3 years. In the landmark study by Vleggaar et al. (2011, Journal of the American Academy of Dermatology, n=278), patients demonstrated sustained facial volume correction through 25 months. The collagen produced is the patient's own type-I collagen — the product itself degrades completely in 9–12 months, and the collagen persists after that.

Most patients require 2–3 treatment sessions spaced 4–8 weeks apart. Results are not immediate — patients typically see gradual improvement beginning at 3 months and peaking at 6 months.

When to Choose Biostimulator vs Filler

Choose a biostimulator when treating large surface areas (temples, cheeks, jawline, body), when the patient is comfortable waiting 3–6 months for peak results, when the goal is natural gradual improvement, and when the patient wants to invest in long-term collagen instead of repeated maintenance sessions.

Choose an HA filler when precise immediate volume is needed (lip augmentation, acute facial asymmetry), when reversibility is important (first-time patient, high-risk anatomy), or when the treatment area is small and targeted.

Many experienced injectors combine both approaches — biostimulators for the broad foundation of volume and collagen, HA fillers for precise contouring in specific areas.

What to Know About Bellafill

Bellafill is unique among biostimulators because the PMMA (polymethylmethacrylate) microspheres do not degrade — making it effectively semi-permanent. It is the only FDA-approved filler with a 5-year clinical study, and it holds FDA approval for both nasolabial folds and acne scars. Because it cannot be reversed, it requires a skin test for bovine collagen (the carrier gel) prior to treatment, and patient selection should be conservative.
